Person - centred approach
The Package Of Care (POC) will be designed with you and/or representatives in order to meet your individual current need. The POC can be modified as needs are changing.

Our aim is to reduce the stress and pressure on our clients and their families by focusing on making their lives easier and more convenient. Our support team's aim is to promote independence and dignity while supporting our clients through rehabilitation. Mantain a confortable style of life at home. Our staff work closely with the multidisciplinary team: Community nurses, GP, Physiotherapist, Occupational therapist and support our clients contacting/arranging appointments and follow-ups.
